  • Residents wandering the halls without supervision
  • Reports of residents wandering away from the facility
  • Medication errors such as wrong medication, wrong dosage or untimely/improper dispensation
  • Improper or unnecessary restraint of patients
  • Emotional changes in your loved one such as becoming withdrawn or unhappy
  • Lack of hygienic care such as dirty clothes, unbrushed teeth or body odors
  • Inadequate lighting, cluttered hallways or broken devices
  • Staff that appear consistently frantic, stressed or tired
  • Strained relationships between staff and residents
